RE: If you get your prescriptions by mail, please read this important notice. Your immediate attention may be required.

Dear State of Texas GBP Participant:

As you know, Community First Health Plans began using the prescription drug services of National Pharmaceutical Services (NPS) on February 1, 2008. We feel that you and your covered dependents will enjoy the benefits and ease of using NPS.

With the change in pharmacy benefit managers also came a change in the mail-order provider. Under the new program you now are able to receive your medication(s) via the NPS mail order service, Integrated HMO Pharmacy.

To get started using Integrated HMO Pharmacy for your mail-order prescription needs, you will need to fill out the mail-order switch form. Please include the following:

  • Your member information in the top section of the form
  • Payment information in the middle section
  • The list of medications you are currently taking
  • The prescribing physician's name and phone number in the bottom section.

Integrated HMO Pharmacy will contact your physician directly to obtain new prescriptions for your medications. If we are unable to complete this process, NPS will call you so you can enlist your physician's office to help make the switch.

The mail-order benefit applies mainly to maintenance medications. If you are in need of an acute or short-term medication (such as antibiotics) you may continue to use your local pharmacy.

If you have any questions regarding your prescription drug benefit program, contact the NPS Customer Service Center at 1-800-546-5677.

Sincerely,

National Pharmaceutical Services & Community First Health Plans
